Patient Instructions

Patients must be referred to the clinic by a doctor (family doctor, walk-in-clinic physician, or specialist).

Colonoscopy Appointment Instructions:

When booking your appointment you will be told to use one of two bowel preparation instructions. Click here to view and print a copy of the appropriate Patient Document Package:

This sheet contains directions to the clinic, various appointment details, the associated cost of your visit (Fee for Uninsured Services), and bowel preparation instructions. Please also view the newsletter called "How to Improve Your Colonoscopy Experience."

Failure to follow the preparation instructions may result in cancellation of your procedure. The food restrictions and bowel preparation described are essential to ensure that your bowel is empty at the time of the colonoscopy. The preparation must be started the day before your procedure is scheduled to take place. If you have questions or concerns please call the clinic for clarification.

For information on dietary restrictions please read the documents below:
Low-Fiber Diet for Colonoscopy Preparation
Clear-Liquid Diet for Colonoscopy Preparation

If you have difficulty with the English language you should bring a friend or family member to the clinic with you to act as an interpreter.

Informed Consent:

Before your appointment is booked at the VEC you must read the consent form and agree to the procedure. Prior to the procedure you will be required to sign the consent form that informs you of the risks and benefits of the procedure. If you have difficulty with the English language you should bring a friend or family member to act as a interpreter for you.

If you have also been scheduled for an Upper Endoscopy click here to view and print a copy of the Gastroscopy Informed Consent.

Medical History:

In order to proceed with the colonoscopy appointment booking we must receive a copy of the Patient Medical History form. Patients will not be accepted for the procedure if they have significant medical issues that would put them at above average risk to have the procedure done outside of a hospital setting.
